45 yrs old patient who sustained open segmental fracture of right leg, treated at a tertiary hospital elsewhere with external fixator 6 months back presented to us with severe infection, pouring pus. Patient insisted for amputation to get rid of the problems he faced. He was counselled for salvage procedure with Ilizarov. Patient underwent extensive wound debridement, removal of necrotic bone of about 15 cms and application of Ilizarov fixator. 2 weeks later he underwent proximal tibial corticotomy and distraction was started. The infection gradually settled down, the bone regeneration was good, the fracture healed well over 10 months. 15cms of new bone formation achieved. The patient was mobilised from the next day of the surgery with full weight bearing on the limb. Fixator was removed in a year. Patient is back to his work and he is grateful to the institution for salvaging the limb.
